I lost my dog's Registration and Pedigree Certificates. How do I obtain another one?
You can download the "AFFIDAVIT FORM" from our website. Then you will have to fill-up and sign the form and send it to the office along with your payment for the Duplicate Copy amounting to Php110.00

I just lost my dog's PCCI certificate that I obtained from the previous owner and I did not transfer the ownership into my name. How do I obtain another certificate?
If you lost your dog's certificate and the dog was not yet transferred into your name, the dog's recorded owner will need to fill-up the PCCI Affidavit Form and the Supplemental Transfer Statement Form. Please include the following information:

    • Dog's name, registration number and breed
    • Recorded owners name, address and signature
    • Php110.00 processing fee (you can either pay by check or money order made payable to the PCCI.
    • Php250.00 transfer of ownership
    • Php33.00 supplemental transfer

If they do not have the dog's name and registration number please provide us with the following in its place:

    • Names and registration numbers of the sire and dam, and breed
    • Date of birth of the litter
    • Recorded owner's name and address

When the required information is received a new certificate will be mailed to you as the new owner.

Please remember a duplicate certificate can only be requested by the current owner on PCCI record.
